Overview of Essential Thrombocythemia and possible complications from chronic elevated mold exposures, mitochondrial dysfunction and imbalanced neurotransmitters. Discussed depressed microbiome, low neurotransmitters and related brain fog/mood issues. Also comments on COMT genes and if a person can be “high catecholamine” even without any COMT SNPs.

    Q & A 06.28.2024

    Dr. Spencer

    Patient: 12 year old boy

Symptoms: brain fog, depression, body aches, headaches, armpit itching, a history of UTI’s and fatigue.

History: His mother says he is a smart, good natured boy with an unusual history of strep. infections. At 1 yr old he had an ear infection and was put on an antibiotic. At 2 or 4 yrs old he had bruising and red spots all over his body. The urgent care doctor said he had “strep all throughout his body,” and he was again put on an antibiotic. At age 6, he had unusual liquid filled blisters on his ears, as well as flat itchy spots all over his arms and legs. His mother used bentonite clay packs and they went away. 1-2 months later, this developed into extreme apathy, headaches, and constant pain, which spread to his wrists and armpits. An MRI revealed inflamed sinuses. A women’s health NP suspected systemic strep and again he was put on an antibiotic. His headache went away immediately. A few years later he visited a natural health, BioMeridian using, practitioner and was told he didn’t have any strep, but he did have neurotoxins in his body. Within the last 3-4 months he reports poor sleep, but attests to sleeping better using a grounding mat this past week. 2 months ago, another biomeridian test revealed strep in his body. Use of the grounding mat and the past 2 weeks’ administration of natural antibiotics and a starch-free diet have lessened his previously-constant-pain significantly. Lately he complains that when the AC hits his legs and feet at night they hurt and cramp up. The brain fog is still rampant and difficult for him to deal with. He says he’s in a constant state of “blah,” which is now sometimes coupled with pain.

He seems sensitive to some nuts and doesn’t handle cheap eggs well. He craves sour things. He is currently eating a clean, mostly starch free and gluten free diet. He has a history of trying the Paleo and Keto diet. His mother has him taking nebulized hydrogen peroxide and colloidal silver, along with Ultrabiotic Daily, Multi-Strain. He also takes vitamin D, vitamin C, omega 3’s, magnesium, oregano, ginger, garlic, zinc and a methylated multivitamin called “Brain Sense”.

His uncle has EDS and the patient’s mother is wondering if his symptoms could be attributed to that.

Attached is an OAT. If it asks for a password it is, “YES”.

    PROTOCOL:

Stop taking garlic, oregano as it would likely suppress his bacteria even more.

Saccharomyces boullardi – 1-2 capsules per day (consider Ultrabiotic Defense)

NeuroBalance – 4 BID (4 w/ breakfast, 4 w/ lunch) – or – if more support is needed you would give L-Tyrosine 1000mg TID + D,L-Phenylalanine 500mg TID and Methyl Complete 2-4 per day as a therapeutic trial for raising Dopamine. I would give him 4 weeks on either protocol before making a judgement on effectiveness.

    MitoRecharge – 2 BID

Glutathione Support – either take orally Liposomal Glutathione 2 dropperfulls 2x per day – or – consider using glutathione nebulizer (instructions below) treatment.

    Case 1)

I do not like the term “circle back” lol but needing some clarification from the OAT I submitted last week. You recommended:

Fungal protocol, Mito Recharge, Cellular Energy, glutathione, NAC, ALA, and Bone Support.

I saw this patient this week for her OAT results and treatment plan. She is not good; frail, difficulty walking, cannot sleep, resting tremor in dominant hand. She has difficulty tolerating supplements and will only agree to taking one new thing at a time. Very fixated on her reactions to ANYTHING.

I do not think she can tolerate the SIBO/SIFO detox protocol at the current time due to being extremely weak. She lives in a 1908 farmhouse which has been treated for mold in the ductwork in February 2024. I started her on HCL, Pan5X, vitamin D, and glutamine powder. Mito supplements but NutriDyn out of stock on Mito Recharge.

Bone Support, Stress essentials relax, and sulforaphane for oxalates.

Is it OK wait on the gut protocol and treat everything else or are her mito problems due to mold? Order of treatment in this situation?

You have to wait and work around their sensitivities. Look into LIMBIC SYSTEM retraining like the GUPTA PROTOCOL. These patients need to work on their LIMBIC SYSTEM that is trying to keep them safe, but it is doing it in an unhealthy way.

    Case 2)

Is it possible to have a COMT158 -/- and be a high dopamine individual?

It is possible because epigenetics and other factors could interfere and inhibit the breakdown of dopamine. That would give similar symptoms even though the COMT pathway is genetically working faster. Phenols in the gut, inflammation, co-factor availability, polyphenols in the diet (some interact with COMT), look into the Feingold Diet and see if that provides any support or improvement in a case like this.

How does a COMT 158 +/- individual present? He is also MTHFR C677t +/+.

Typical depression phenotype, when unhealthy and poor gut health and diet, likely low dopamine and low motivation, fatigue, brain fog, evidence of slow frontal lobe function.

    Dr. Leah

    REVIEWED OAT – High fungal markers x3, w/ very high Arabinose (Candida albicans); high Mitochondrial markers x3 (#24-26); high Ketones x4; Significant dopamine dominance w/ relatively low adrenalin;

REVIEWED MYCOTOXIN – high fungal markers x3 despite low mold environment and previous treatment to detoxify mold.

Performed a full SIFO and gut protocol, including Diflucan and Nystatin, in an alternating fashion. She started treating

    PROTOCOL:

Nebulizer treatment – use glutathione 600mg QD up to TID for about 20 minutes

    GLUTATHIONE NEBULIZER

You may purchase the glutathione capsules for the nebulizer here: https://www.theranaturals.com/natural-supplements/1-reduced-l-glutahtione-plus-enhanced-absorption-ultra-purity-grade.html.

As a precaution, start with 1 capsule dissolved in 5 ml of distilled water once daily. If you notice any shortness of breath or difficulty breathing, discontinue use. Otherwise, you may gradually increase dosage up to 3 capsules dissolved in 5ml of distilled water once daily for 2 to 4 weeks. After this you may reduce to dosage to an as-needed basis for maintenance.

Nebulizers are inexpensive and can be found on here: http://justnebulizers.com/omron-compair-nebulizer-system-ne-c801.html. We like the Omron model particularly. I have attached a doctor’s letter you will need in order to purchase the nebulizer.

Riboflavin-5-Phosphate – Thorne has the only professional Riboflavin, I would take 1 TID for one bottle, then 1 QD for another bottle.

    Taurine/Glycine/Sulforphane/Milk Thistle/Ganoderma/Astragulus/
